1. Hypertension (HTN)

Nearly half of U.S. adults have hypertension.
Stages:

  • Normal = less than 120 over less than 80 Elevated = 120 to 129 over less than 80

  • Stage 1 = 130 to 139 or 80 to 89 Stage 2 = 140 or higher or 90 or higher Crisis = 180 or higher or 120 or higher (urgent/emergency)

Goal: Keep blood pressure under 130 over 80 for most patients.
Prevention: DASH or Mediterranean diet, less than 2.3 grams of sodium daily, 150 minutes per week of exercise, limit alcohol.

2. Obesity

Over 40 percent of adults have obesity. BMI Classes: Class 1 = 30.0 to 34.9 Class 2 = 35.0 to 39.9 Class 3 = 40.0 or higher

Prevention: Plant-based meals, high fiber intake (25–30 grams per day), daily physical activity, and behavioral support.

3. Prediabetes and Diabetes

Prediabetes affects about one in three adults.
Prediabetes: A1c between 5.7 and 6.4 percent, fasting glucose between 100 and 125 milligrams per deciliter, or two-hour glucose 140 to 199.
Diabetes: A1c 6.5 percent or higher, fasting glucose 126 or higher, or random glucose 200 or higher with symptoms.

Prevention: Weight loss of 5 to 10 percent, 150 minutes per week of exercise, and reduced sugar intake.

4. Chronic Kidney Disease (CKD)

Affects about 14 percent of adults; most are unaware.
Stages:  

Stage 1 – eGFR 90 or higher Stage 2 – eGFR 60 to 89  

Stage 3a – eGFR 45 to 59 Stage 3b – eGFR 30 to 44
Stage 4 – eGFR 15 to 29 Stage 5 – eGFR less than 15 (kidney failure)

Albuminuria: A1 less than 30 milligrams per gram A2 between 30 and 300 A3 more than 300

Prevention: Control blood pressure and glucose, use ACE inhibitors or ARBs, avoid NSAIDs, stay hydrated.

5. Hyperlipidemia and Atherosclerotic Risk

About 11 percent of adults have total cholesterol above 240.
Prevention: Mediterranean-style diet, soluble fiber, reduce trans fats, regular exercise.

6. Coronary Artery Disease

Roughly 5 percent of adults affected.
Prevention: Manage blood pressure, cholesterol, and diabetes, stop smoking, exercise regularly, and maintain a plant-rich diet.

7. Osteoarthritis (OA)

Affects about one in five adults.  

Grade 1 – Doubtful osteophytes, no joint space narrowing  

Grade 2 – Definite osteophytes, possible narrowing
Grade 3 – Multiple osteophytes, definite narrowing, some sclerosis  

Grade 4 – Large osteophytes, marked narrowing, deformity present

Prevention: Weight control, physical therapy, low-impact exercise, anti-inflammatory diet, and topical NSAIDs.

8. Chronic Pain

Affects over 20 percent of adults. Pain Scale: Mild = 1 to 3 Moderate = 4 to 6 Severe = 7 to 10

Treatment: Function-focused therapy, graded movement, stress and sleep management, non-opioid medications.

9. Anxiety Disorders

About 19 percent of adults yearly.
Treatment: CBT, relaxation training, sleep hygiene, SSRIs or SNRIs if needed.

10. Depression

Affects about 8 percent of adults yearly.

11. Asthma

About 8.6 percent of adults.
Control means symptoms fewer than two days per week, no limits on activity, and no night awakenings.

12. COPD

About 4 percent of adults.

17. Alcohol Use Disorder

Affects about 28 million people aged 12 or older.

18. Nonalcoholic Fatty Liver Disease (MASLD / NAFLD)

Affects about 30 percent of adults. Prevention: Weight loss of 7 to 10 percent, Mediterranean diet, physical activity, and low sugar intake.

19. Low Back Pain

Affects nearly 40 percent of adults at least once. Treatment: Maintain movement, posture training, avoid prolonged rest.

Common in postmenopausal women and older men.
